Clinical documentation · NHS emergency departments

The wait is the only unhurried time in A&E.

trAIgo holds a structured clinical conversation with patients while they sit in the waiting room, then hands the treating clinician a clerking-ready history — written up before the consultation even begins.

In development. Built by an NHS emergency medicine doctor and a software engineer, for the department one of them works in.

So tell me why you've come in today?
I've got pain in my knee
When did the pain start?
What were you doing when the pain started?
i was mountain biking with a friend, then i had a crash.
Were you able to put weight on your leg immediately after the crash?

The problem

Emergency departments spend their scarcest resource on their most predictable task.

Patients wait, unattended

Hours pass in the waiting room during which nothing about the case is being gathered. Dead time for the department, anxious time for the patient.

Clinicians document instead of deciding

A large share of every consultation goes on taking a history that follows a largely predictable structure — then writing it up a second time.

Triage doesn't reach the note

Nurse-led triage is built to prioritise, not to clerk. The doctor still starts the history from scratch, and the patient repeats themselves again.

How it works Three steps

From waiting room to clerking-ready history.

The patient installs nothing. The clinician never loses control of the note.

01  SCAN

On arrival

The patient scans a QR code in the waiting area and A&E Helper opens in their own browser. No app to install, no account to create.

02  CONVERSE

While they wait

A structured conversation works through onset, site, character, severity, timing, aggravating and relieving factors — and the red flags that matter for that complaint. It asks real follow-ups and doesn't repeat itself.

03  HAND OVER

Straight to the clinician

When the history is done, the patient generates a code and shows it. The clinician scans it on their own phone and the summary opens — as a paragraph for the note, or itemised for review.

The product

Built and working today.

These are real screens from the current build, running a real consultation end to end.

The patient view: a chat asking about knee pain, when it started, and what the patient was doing at the time.

The conversationAdaptive questioning that follows the complaint wherever it goes.

The patient view showing a QR code with the caption: when ready, show this to a clinician.

The handoverThe patient generates a code. The clinician scans it. No transcription step.

The clinician view: a structured summary with presenting complaint, history of presenting complaint, and red flags highlighted in red.

The summaryPresenting complaint, history, and red flags — ready to read or copy.

Screens from the current build. Test data throughout — no real patient information.

Scope & safety

A clinical assistant. Deliberately not a diagnostic one.

The boundary is the product. Everything below is out of scope by design, not by omission.

It does not diagnose
It does not give medical advice
It does not triage or prioritise patients
It does not write to the patient record
It does not replace clinical judgement
It does not act on what it hears

What it does is gather, structure and hand over. Red flags are surfaced for the clinician, never acted on by the system. A clinician reads every summary, checks it against the patient in front of them, and decides what enters the record.

Governance

Regulated before deployed.

trAIgo is classified as medical software. We treat certification as a prerequisite to clinical use, not a formality that follows it.

01

MHRA certification under UK MDR

In progress. No deployment into live clinical use ahead of it.

02

Independent penetration testing and security audit

Planned as part of the same certification process, alongside a full information governance review.

03

Sessions expire and are deleted

Patient consultations are short-lived by design. Once a session has expired it is removed, not archived.

04

Clinician access is authenticated, with no shared passwords

Clinicians sign in with a single-use link sent to their own address, which expires after fifteen minutes.

05

NHS Data Security and Protection Toolkit

Alignment with the DSP Toolkit and NHS information governance standards is a design constraint, not a later addition.

06

European hosting on Google Cloud

Currently hosted in Google Cloud's European region, with UK data residency for any Trust deployment.
